Caring for complex patients requires a thoughtfully coordinated and passionate care team.
Our vision emerged from a shared frustration with a healthcare system that too often prioritize volume, indifference, and fragmentation over the quality, compassion, and collaboration our most vulnerable patients deserve.
We recognized that clinicians alone cannot build an effective post-acute and long-term care medical group without understanding the realities of facility operations – just as facility operators cannot fully appreciate the complexities of modern medical care.
For this reason, our organization was intentionally formed by physician leaders, facility owners, and nationally recognized operators – aligning clinical excellence with operational expertise.

Many people are familiar with hospitals named after beloved saints, but fewer know the story of St. Giuseppe Moscati – the physician whose life inspires Moscati Medical Group.
St. Moscati embodied both scientific excellence and extraordinary compassion. He cared for patients others avoided, risked his own life to save others, served the poor, and lived medicine not merely as a profession, but as a true calling.

Chief Executive Officer
Kimberly Green-Yates

Kimberly Green-Yates is the Founder and Chief Executive Officer of Sage Integrated Health, a senior living organization focused on improving care for aging adults across the full continuum – from independent living to skilled nursing. With more than 35 years of experience in senior care, she began her career as a certified nursing assistant and advanced to executive leadership, giving her a unique perspective that bridges frontline clinical care and operational strategy. A medical speech-language pathologist specializing in cognitive disorders, Kimberly previously served 13 years as a Chief Operating Officer overseeing senior care operations while consulting nationally on quality improvement and dementia care. She is an international speaker, published author, and recognized leader in aging services, serving on multiple boards and statewide initiatives. In recognition of her contributions to the field, she was inducted into the McKnight’s Women of Distinction National Hall of Honor.

Ryan Reynolds
DO
Dr. Ryan Reynolds is an internal medicine physician focused on geriatrics, hospice, and palliative care. He earned his medical degree from the University of North Texas Health Science Center and completed his training at Houston Methodist Hospital, where he developed a strong foundation in complex medical care. He began his career as a hospitalist in the Texas Medical Center and later served as a medical director within the Ascension St. John Health System in Tulsa, Oklahoma. After witnessing firsthand the consequences of inconsistent physician presence in post-acute and long-term care settings, he now focuses his clinical practice in these
environments, driven by a commitment to improving outcomes for older and medically vulnerable patients through more consistent, accountable, and compassionate physician-led care. Dr. Reynolds is dedicated to building care models that emphasize clinical excellence, dignity, and continuity across care settings.
Chief Financial Officer
Mike Yates
CPA

Mike Yates is a healthcare finance executive with nearly 30 years of experience across physician- led practices, hospitals, multi-state service organizations, and the insurance payer environment. His work has focused on financial strategy, operational discipline, and sustainable growth within complex healthcare systems. Most recently, Mike spent five years in the dental service organization sector, helping lead financial performance and expansion for multi-state operations. Prior to that, he served 11 years in senior financial leadership roles at two large oncology organizations, supporting physician practices through growth, reimbursement complexity, and operational scaling. Earlier in his career, he spent eight years as Vice President of Finance at a large insurance brokerage, providing him with a unique perspective across both provider and payer environments.
